DATA SUBJECT APPLICATION FORM

Under the Law on the Protection of Personal Data No. 6698 (hereinafter referred to as the "Law"), data subjects, defined as relevant persons, (hereinafter referred to as the "Applicant"), are granted the right to make certain requests regarding the processing of their personal data in Article 11 of the Law. Saldos Ticaret Anonim Şirketi acts as a data processor within the scope of this form, and the data controller is Transmed Gıda Sanayi ve Ticaret A.Ş.; applications are received by Saldos as a data processor and evaluated on behalf of the data controller.

Applications to our company regarding these rights must be submitted to Saldos Ticaret Anonim Şirketi in writing or electronically, in accordance with Article 13, paragraph 1 of the Law and the relevant provisions of the Communiqué on the Procedures and Principles for Application to the Data Controller, using the methods outlined below.

Application Methods

For applications to our company within the scope of the Law on the Protection of Personal Data, it is mandatory to include the applicant's name and surname, signature if the application is written, Turkish Republic ID number for Turkish citizens, nationality, passport number or ID number if any for foreigners, residential or business address for notification, email address for notification if any, phone and fax number, and the subject of the request, in accordance with Article 5/2 of the Communiqué on the Procedures and Principles for Application to the Data Controller, published in the Official Gazette dated 10.03.2018 and numbered 30356.

1-Written Applications:

Written applications to our company must be submitted by hand or by proxy with a notarized power of attorney showing authorization to apply for the rights specified in Article 11 of the Law, or by mail through a notary to Cumhuriyet Mah. Akeller Cad. No:107 Söke/AYDIN, with a wet-signed copy of this "Application Form" filled completely and containing the above information.

2-Electronic Applications:

Electronic applications to our company must be made by sending this "Application Form," which is fully completed and contains the above information,

• Signed with a "secure electronic signature" certificate, as defined in the Electronic Signature Law No. 5070, or a mobile signature, to our company's Registered Electronic Mail (KEP) address saldos@hs01.kep.tr,
• Sent from the relevant person's email address, previously notified to our company and registered in our company's system, to kvkk@saldos.com.tr,

We will respond to your applications submitted to us within the shortest possible time, and no later than thirty days from the date your request reaches us, depending on the nature of the request, in accordance with Article 13, paragraph 2 of the Law. Our responses will be delivered to you in writing or electronically based on your request, as per the provisions of the same article of the Law.

This application form has been prepared to identify your relationship with Saldos Ticaret Anonim Şirketi, to determine any personal data processed by Saldos Ticaret Anonim Şirketi completely, and to respond to your relevant application accurately and within the legal period. In accordance with the Law and within the scope of the Communiqué on the Procedures and Principles for Applications to the Data Controller (“Communiqué”), the date your application reaches us will be considered the application date. Your submitted applications will be accepted by us as a result of our evaluations; if there is incomplete information in your submitted application, your request may be rejected by us. To eliminate legal risks that may arise from unlawful and unfair data sharing and especially to ensure the security of your personal data, Saldos Ticaret Anonim Şirketi reserves the right to request additional documents and information for identity and authorization verification. In this regard, we will inform you and send you the information we request to complete your missing information.

If the information regarding your requests submitted within the scope of the form is inaccurate or outdated, or if an unauthorized application is made, Saldos Ticaret Anonim Şirketi accepts no liability for requests arising from such inaccurate information or unauthorized application.

Your information submitted with the application will be recorded within the scope of this form by Saldos Ticaret Anonim Şirketi, and your data will be erased by us with the necessary technical and administrative measures taken when the deletion periods determined based on data categories expire. If your application results require an additional cost within our scanning and report generation process, the fees specified in Article 7 of the Communiqué may be charged to you.
